Community Assessment and Plans

The Clatsop County Community Assessment, Community Health Improvement Plan (CHIP) and Public Health 2013-14 Strategic Work Plan are the result of a collaborative effort among the public, stakeholders and key leaders in public health to assess the state of health in our community and outline strategies to work together to improve the health of Clatsop County.

Clatsop County Community Assessment is a collection and analysis of information on the characteristics, trends and needs of children and families in our county.

Community Health Improvement Plan (CHIP) is a community roadmap and decision-making tool that identifies four priority objectives for improving health and quality of life in Clatsop County within the next five years.

Public Health 2013-14 Strategic Work Plan lays out specific activities designed to increase health literacy, strengthen public health infrastructure, build partnerships and promote accountability.

These documents were developed as part of the Public Health Department’s goal of achieving accreditation through the Public Health Accreditation Board, a voluntary national program to establish standards for local and state health agencies.
